First Visit:

To ensure you have the most convenient and useful visit, please take a moment to review some of the suggestions below.​

Once checked in, Dr. Waddell's assistants will greet you and prepare you for your visit. This will include various forms and questionnaires. 

​

Most patients will require X-rays prior to seeing Dr. Waddell.  In many cases, outside X-rays are not the adequate views for Dr. Waddell to best evaluate your condition. Please let our staff know if you have had recent X-rays and we can determine if new ones are necessary.  Please bring all previous imaging with you in the form of a CD.

What time should you arrive for the Appointment?
  • Please arrive 15 minutes before your first appointment so that you can fill out our office forms and questionnaires.

 
Please bring these items with you for the initial visit:
  • Bring your insurance card.

  • If you have seen another physician for the same problem, please bring your X-rays, MRIs, and CT Scans. Please bring CDs of images if you have them.

  • Your medications list and herbal supplements.

  • Your medical records along with a list of your past medical history.

  • Your surgical and operative reports if pertaining to the reason for visit.

  • A list of allergies and any reactions you have to medications

  • Any questions or concerns you have about your current condition.

 

In the Office:

Once X-rays are complete, you will see Dr. Waddell in the clinic.

Dr. Waddell will come up with a treatment plan based on your symptoms, radiographic images (X-ray, MRI, CT) and your and his preferences. You are a vital part of the decision making when it comes to the treatment plan. 

​

If surgery is scheduled, you will have appointments made for the pre-operative evaluation with the IPA Pre-Admit Testing to ensure you will be as safe as possible for your surgery. It is extremely beneficial to see your PCP and other medical doctors you have prior to surgery.
